Are you a dynamic and motivated individual with strong leadership skills and a professional nursing registration? If so South Manchester community services are is seeking to recruit a Senior Practitioner to join our vibrant teams. The post holder will provide effective clinical/nursing leadership for the North Mersey CMHT based at Kingslea House, developing and maintaining a high quality of service delivery. Offering nurse led clinics for both pre-screening appointments and to review any cases which require more robust follow up to establish suitability for the CMHTs or senior input. Working with the MDT to manage the 28 day referral target and ensure that referrals are seen and closed in a timely fashion whilst also providing safe decision making. Providing support within the management team to ensure oversight and efficient running of the team within best practice. The post holder will: 1. Provide clinical leadership, supervision, support and consultation to staff in the CMHT, in line with clinical pathways and as outlined within the operational procedure. 2. Take a lead clinical role in identified clinical pathways within the service 3. Manage and evaluate caseloads in line with relevant clinical pathway, as outlined within the operational procedure and will include: • Assessment • Formulation • Therapeutic intervention as required • Signposting • Close working with all colleagues • To be responsible for a complex caseload Deputise for the Team Manager in his/her absence and take on any delegated management duties as required. To participate in the bronze on call rota this is subject to approval.
